Nondiscrimination

Potomac Health complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, sex, sexual orientation, or gender identity.

Language and accessibility assistance

We provide free aids and services to people with disabilities and free language assistance to people whose primary language is not English. Call (732) 691-0507 to request help.

Your rights as a patient

You have the right to respectful care, to understand your diagnosis and treatment options, to know the cost of services before you receive them, to access your medical records, and to end your membership with 30 days written notice.

Concerns and grievances

Please share any concern directly with the practice so we can resolve it. You may also contact the Virginia Department of Health Professions or file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights.
